Position Highlights:

Responsibilities:
- Coordinates the continuity of patient care with assigned patients and families following hospital admission, discharge, and ER visits.
- Supports patient self-management of disease and behavior modification interventions.
- Provide triage to joint and spine patients, following nursing guidelines, protocols, and physician direction.
- Ensures Western Orthopedics & Sports Medicine and Grand Valley Spine meet the standards of The Joint Commission (TJC).
- Directs preparations and ongoing monitoring of accreditation standards.
- Maintains and monitors quality matrix and files in compliance with TJC.
- Incorporate factors related to safety, effectiveness, cost, and impact on practice in planning and delivering patient care in regard to resource utilization.
- Maintains data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Center of Excellence for Orthopedic and Spine patients as directed by The Joint Commission.
- Evaluates clinical care, utilization of resources, and development of new clinical tools, forms and procedures related to The Center of Excellence.
Requirements:
- Bachelor's degree in nursing
- At least three (3) years of related experience in case management and/or utilization management, or similar experience required.
- Experience in orthopedics or spine preferred.
- Certification in Case Management preferred or willing to obtain within three (3) years of employment
- Registered Nurse (RN) in the State of Colorado.
Compensation:
- $39.00 - $44.85 per hour, depending on education and experience.
- Bonus: $5,000
- Discretionary bonuses, relocation expenses, merit increase, market adjustments, recognition bonuses, and other forms of discretionary compensation may be available.
