The Licensed Practical Nurse (LPN) plays a key role in delivering high-quality, patient-centered care in a busy outpatient oncology and infusion center setting. Under the supervision of a Registered Nurse (RN), Advanced Practice Registered Nurse (APRN), or provider, the LPN performs clinical and administrative tasks within the scope of supportive and restorative care. This includes health education, medication administration, specimen collection, and assisting with procedures. The LPN contributes to all phases of the nursing process and ensures safe, efficient care delivery.

Qualifications

Minimum Requirements:

• Current LPN license in the State of Connecticut.

Experience:
• Competency in phlebotomy, bladder scanning, EKGs, and emergency response.
•Oncology or outpatient care experience preferred.
•Strong interpersonal, organizational, and communication skills.
•EMR proficiency (Epic experience preferred).

Job Description : The Wellness Specialist (LPN/RN) is responsible to provide clinical leadership support in communities where a clinical leader may not be present. The Wellness Director Specialist will provide temporary coverage for the overall implementation, delivery, and coordination of resident clinical services at the community. Directly supervises all employees of the nursing department. This position is responsible for the overall direction, coordination, and evaluation of this department. This person also carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. The Wellness Specialist (LPN/RN) is responsible for assisting in onboarding and training new Wellness Directors. This individual will support the following states: MA, MD, and NY. Responsibilities : Involves being completely familiar with the state regulations governing Assisted Living and ensuring compliance in department by regularly reviewing regulations checklist. Performs timely and accurate resident assessments utilizing assessment tool, and communicating changes to the Executive Director for billing purposes Manages expenses within given budget parameters utilizing spend-down sheets. Provides oversight with medication management to assure all MAR's are correct and that the residents receive medications timely and accurately. Working with consultant pharmacist as needed to assure community is in compliance. Ensures all charting is performed in a timely manner and in accordance with state regulations. Provides appropriate guidance for team members by planning work schedules, assigning work duties while on shift, and directing the work on a daily basis. Provides on-call backup to community when not on duty. Maintains confidentiality of all pertinent personal or health information concerning residents and staff. Ensures that any incident reports are completed properly and promptly. Other responsibilities as assigned by the Regional Clinical Manager Qualifications: Qualified candidates must be currently licensed as a RN or LPN with a minimum of 1-year experience in senior housing and/or geriatric nursing. Minimum 2 years’ experience working as a Director of Nursing or Wellness Director with a demonstrated success in leading nursing teams to meet successful resident outcomes. Candidate must be able to travel to various communities within the assigned market. Prior management experience preferred. Candidate must either be certified or be able to become CPR and First Aid Certified within the first 90 days of employment. Candidate must also be able to read, write and speak the English language and be able to communicate effectively with residents, families, physicians, and team members.
