He/she has specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and he/she may work independently or as part of a treatment team. Initial patient encounters are in response to consultation requests, and the initial consultation may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family. The care plan advanced by the NP Provider aims to prevent or mitigate illness burden for the patient and family.
