1. Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations. 2. Performs assessments on Valve Clinic patients. 3. With Valve Clinic Team, initiates stroke orders and treatment plan to provide acute care, including on-going assessment, routine procedures, documentation in patient chart, family/patient counseling, education, and procedural & discharge planning. 4. Demonstrates implementation skills in accordance with EMH Standards of Care, policies, procedures, and Nurse Practice Act. 5. With the supervision of a Physician, meets with patients in the Valve Clinic Setting. 6. Demonstrates ability to change intervention strategies when expected outcomes not met. 7. Consistently documents in accordance with EMH policies and procedures. 8. Assesses and analyzes outcome Measures for continued Valve Clinic Progra. 9. Demonstrates competency in the operation of equipment specific to clinical area. 10. Provides accurate and precise reporting of patient condition and changes in a timely manner to physicians. 11. Identifies educational needs and provides comprehensive education pertaining to Valve Clinic Program. 12. Collaborates with nursing management, physicians and the Education Department to identify gaps in processes, develop and implement staff education related to Valve Clinic Program. 13. Completes age specific competency annually. 14. Coordinate research, collect and collate data and report results. Responsible for NCDR TVT Registry. 15. Complies with departmental plan to meet budget goals. 16. Demonstrates knowledge of Department of Nursing and Joint Commission required quality indicators, performance improvement and patient safety goals. 17. Develops and runs Valve clinic in coordination with Cardiac Services Director. 18. Assists with Valve Clinic Conference held monthly. 19. Performs other duties as assigned.Related Jobs
