Essential Responsibilities

With Valve Clinic Team, initiates stroke orders and treatment plan to provide acute care, including on-going assessment, routine procedures, documentation in patient chart, family/patient counseling, education, and procedural & discharge planning. 4. Demonstrates implementation skills in accordance with EMH Standards of Care, policies, procedures, and Nurse Practice Act. 5. With the supervision of a Physician, meets with patients in the Valve Clinic Setting. 6. Demonstrates ability to change intervention strategies when expected outcomes not met. 7. Consistently documents in accordance with EMH policies and procedures. 8. Assesses and analyzes outcome Measures for continued Valve Clinic Progra. 9. Demonstrates competency in the operation of equipment specific to clinical area. 10. Provides accurate and precise reporting of patient condition and changes in a timely manner to physicians. 11. Identifies educational needs and provides comprehensive education pertaining to Valve Clinic Program. 12. Collaborates with nursing management, physicians and the Education Department to identify gaps in processes, develop and implement staff education related to Valve Clinic Program. 13. Completes age specific competency annually. 14. Coordinate research, collect and collate data and report results. Responsible for NCDR TVT Registry. 15. Complies with departmental plan to meet budget goals. 16. Demonstrates knowledge of Department of Nursing and Joint Commission required quality indicators, performance improvement and patient safety goals. 17. Responsible for coordinating and supervising nursing staff and Medical Assistants. Monitors the performance of competency, gives verbal and written input for annual performance evaluations, as well as assisting the manager regarding the compliance of policies and procedures. Functions as a liaison for medical staff and other customers for the manager when not present. Contributes to the development of policies and procedures, performance improvement plans. Responsible for general oversight of staff orientation and development; optimal provision of quality patient care. Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations. 2. Monitors department specific standards of care/practice. 3. Assists with collection of data for Nursing P.I. projects. 4. Assists in monitoring unit readiness in Joint Commission, California and Federal standards. 5. Ensures daily assessment of unit equipment to maintain quality controls (i.e. POCT’s, Med Refrigerators, where applicable. 6. Provides coaching for staff as necessary. 7. Rounds with staff members to evaluate care and services given to patients. 8. Facilitates availability of adequate material resources and supplies. 9. Assists control of expenditures within the department. 10. Assesses shift staffing needs consistent with patient mix, census and acuity levels. 11. Sets priorities appropriate to daily patient/physician dynamics. 12. Assures patient acuity classification is completed. 13. Maintains communication and resolves issues regarding shift activities. 14. Coaches and educates staff Nurses regarding clinical questions. 15. Retains clinical skills for execution of patient care as necessary. 16. Collaborates with staff regarding physician notification. 17. Assists with performance evaluations in a timely manner. 18. Promotes efficient patient care throughput unity in the Urgent Care Clinics. 19. Serves as clinical and administrative resource to staff. 20. Balances employee resources effectively to meet clinical needs and monitors daily staffing to meet budget/financial goals by flexing staff as necessary. 21. Performs other duties as assigned.
