Reports To: Department Director


Job Summary:

Provides direct and indirect patient care services that meet the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations. Maintains regulatory agency requirements, nursing and hospital policies, procedures and standards. Communicates with physicians and team members about changes in patient’s clinical condition, including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. Additionally, is able to perform general nursing duties in all departments with adequate supervision.


Demonstrates Competency in the Following Areas:

  • Demonstrates ability to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes pediatric, geriatric and the general patient population.
  • Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical condition.
  • Follows the seven (7) medication rights and reduces the potential for medication errors.
  • Demonstrates ability to revise plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness.
  • Ability to perform waived testing (point-of-care testing) per Clinical Laboratory’s and the resident care unit’s policies and procedures.
  • Ability to interpret results of waived tests; take appropriate action on waived tests results.
  • Formulates a teaching plan based on identified patient learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate.
  • Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills.
  • Demonstrates ability to perform treatments and provide services to level of licensure.
  • Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.
  • Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
  • Identifies and addresses psychosocial needs of patients and family; communicates with Social Service/Discharge Planner regarding both in-hospital and post discharge needs.
  • Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care to the patient population served. Able to interpret data about the patient’s status in order to identify each patient’s age specific needs and provide care needed by the patient group.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Supports and maintains a culture of safety and quality.
  • Coordinates and supervises patient care.
  • Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient’s needs.

  • Maintains a good working relationship both within the department and with other departments.

  • LPN's - Performs other duties as assigned within the LPN scope of practice.
  • LPN's - Coordinates and supervises patient care under the direct supervision of a Registered Nurse.

  • Actively participates in performance improvement and continuous quality improvement (CQI) activities.
  • Complies with all organizational policies regarding ethical business practices.
  • Communicates the mission, ethics and goals of the hospital, as well as the focus statement of the department.

Regulatory Requirements:

  • Graduate of an accredited school of nursing.
  • RN's - Current state Registered Nurse Licensure, within state of Arkansas.
  • LPN's - Current LPN licensure within the state of Arkansas.
  • Current BLS certification.

Language Skills:


• Able to communicate effectively in English, both verbally and in writing.

• Additional languages preferred.


Skills:

• Basic computer knowledge.


Physical Demands:


• Mobility, sight, hearing, verbal communication, changes in temperature, exposure to disagreeable elements, exposure to biological, mechanical, electrical, and chemical hazards. Frequent standing and walking. Lift up to 100 pounds. Carrying up to 40 pounds. Pushing/pulling up to 450 pounds.

Job Summary The Hospice PRN Registered Nurse provides skilled nursing care hospice following physicians’ orders and the established Plan of Care. This position participates in the supervision of Home Health Aides and LPN's. Coordinates and collaborates with the interdisciplinary team ensuring the enactment of the Plan of Care. Job Summary The Hospice PRN Registered Nurse provides skilled nursing care hospice following physicians’ orders and the established Plan of Care. This position participates in the supervision of Home Health Aides and LPN's. Coordinates and collaborates with the interdisciplinary team ensuring the enactment of the Plan of Care. Reports to the Clinical Nurse Manager. Works and schedules a minimum of assigned shifts and call shifts according to PRN contract commitment. This position plays a critical role in supporting Augusta Health’s mission and advancing departmental goals through measurable performance indicators and service excellence. This position contributes to a collaborative, patient-centered environment and helps drive outcomes aligned with organizational priorities. Essential Job Duties Medications Completes 5 Rights with 100% compliance. Knowledge of medications used. Knowledge and use of high-risk medications. Ability to perform calculations for mixtures/solutions. Reconciles patient medications and performs DRR. Evaluation - Evaluate patient outcome: Implements/develops/documents the plan of care to ensure quality and continuity of care and recommend revisions to the plan, as necessary. Updates plan of care based on continuing assessment of patient outcomes. Documentation reflects nursing interventions, patient responses, as well as follow through with patient outcomes. Consults with and educates the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs. Prioritize any needs with the members of the IDT. Provide training to other staff as needed. Evaluation of age specific instruction/teaching is demonstrated and documented through monitoring of the learning process, seeking feedback from patients and family, and return demonstration if appropriate. Prepares clinical and progress notes that demonstrate progress toward established goals. Clinical focus on improving patient outcomes and comfort at end-of-life. Responsible for fiscal utilization of services/supplies per patient. Assess/Plan - Assess and develop individualized needs and plan of care: Applies specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient’s eligibility. Assessments are complete and appropriate for patient’s age, special care needs, and patient's knowledge level, psychosocial, emotional, physical, and spiritual status. Informs physician/provider and other team members of changes in the patient’s needs and outcomes of intervention, while evaluating patient and family response to care. All documentation is submitted in a timely manner and per documentation policy. Communicates patient information as necessary to the interdisciplinary team and other nurses as required. Appropriate referrals are made with evidence of follow-up documentation in the medical record. Personal Growth Assumes responsibility for own personal education and developmental needs. Assesses personal learning needs and initiates methods to meet the identified needs through formal or informal education. Displays positive attitude and flexibility toward work unit. Provides mentoring to orientees and student nurses as assigned. Minimizes absences and reports to work in a dependable manner. Communicates effectively with physicians utilizing SBAR approach. Able to function calmly and promptly while demonstrating appropriate knowledge and skill in emergency situations. Maintains responsible attitude towards management and cost containment. Serves on organizational committees as requested by Clinical Manager. Implement Implement clinical and technical aspects of care in accordance with established standards, policies, and practice guidelines Anticipate patient’s potential complications and need for appropriate preventive measures. Demonstrates strong clinical skills in technical aspects of care, pain management, systems assessment, and patient satisfaction. Demonstrates proficiency in nursing procedures and follows policy. Demonstrates knowledge of proper body mechanics. Disposes of sharps, medication, and infectious waste appropriately per policy. Maintains calm and safe approach with those patients that display violent or aggressive behavior. Practices safe driving. Patient Safety Demonstrates knowledge of patient two unique identifiers. Utilizes SBAR approach to patient hand off communication. Follows policy and procedure in the administration of medication and medication reconciliation. Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease. Abides by policies/procedures for Isolation. Consistently follows CDC guidelines for hand hygiene. Knowledgeable of their role in department emergency preparedness plan. Labels all specimens appropriately in home Required Qualifications Education: Nursing Diploma and/or Associate Degree in Nursing Licensure/Certification: Current and valid Registered Nurse license in the Commonwealth of Virginia or from a state that is part of a Compact Agreement with Virginia CPR-Basic Life Support Healthcare Provider certification (BLS-HCP) Experience: Requires one year of recent Medical/Surgical Experience Driver's License: Yes To drive an Augusta Health Vehicle, must also be 21 years old and Motor Vehicle Record (MVR) must be approved by Risk Management.
