Working on the Age- Friendly Unit means joining a team that is committed to providing excellent care to our geriatric patients. Our patients are typically 65 and above, and we provide our care centered around the 4Ms- “What Mater, Mentation, Medication, and Mobility” as those are pivotable components to providing quality inpatient care to all our patients. Caring for our Med/ Surg geriatric patients can include anything from GI, respiratory, diabetes, cardiac, or infectious complications while stable enough to be on a medical unit. Through providing exceptional care to our patient demographic, it also gives our nursing team an immense amount of knowledge to constantly continue learning throughout their career. We value team involvement for Professional Governance to have bedside nurses' opinions heard at the hospital level as well as our Unit Coordinating Council which is a unit level meeting held once a month where you voice can be heard to change unit practices and concerns. We also encourage teammates who would like to continue their learning and further education in the pursuit of Clinical Ladder Advancement, Professional Certifications, and strive to nurture a unit culture of teamwork, commitment, integrity, and caring.
Our 28- bed Age- Friendly Unit offers groundbreaking 4Ms work to our geriatric patient population, which includes the utilization of Exercise Physiologist, which work with our patients to ensure mobilization as well as preventing deconditioning while hospitalized. We also have a dedicated Geriatric Pharmacist who reviews medications on all our patients to ensure they are given the best medicinal plan possible to prevent delirium while admitted and ensure there are no better options for the medications they take every day, even outside of the hospital setting. We also have Geriatric APPs and a Gerontologist who attend our Interdisciplinary Rounds and can be consulted on our patients and can contribute a specialized medical opinion for our geriatric patients.
