RN Case Manager - Inpatient
About the Role:
The Full Time Exempt RN Case Manager plays a critical role in coordinating and managing patient care within the health care services industry. This position supports the physician and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction through care facilitation and discharge planning functions. Identifies at-risk populations using approved screening tools and follows established reporting procedures. Monitors LOS and ancillary resource use, intervening as appropriate. Coordinates/facilitates patient care progression throughout the continuum. Works collaboratively and maintains active communication in a team setting with physicians, nursing and other members of the multi-disciplinary care team to effect timely, appropriate patient management. Stays current with regulations which impact the care of patients and WCH’s regulatory compliance. Promotes individual professional growth and development and meets mandatory/continuing education. The case manager must be knowledgeable of community resources and alternative care facilities. This position must be able to handle multiple priorities and be self-directed. It requires good organizational skills and analytical thinking.

Minimum Qualifications:
- Bachelor’s degree in Nursing (preferred) or a related health care field.
- Current and active unrestricted Ohio license (RN).
- 5 years of experience in an acute care setting.
- No written disciplinary action in the last 12 months.
- Minimum of two years of experience in case management or a related clinical role within a health care setting.
Preferred Qualifications:
- Certification in Case Management through ACMA.
- Experience working with diverse patient populations and complex care needs.
- Critical care experience.
- Familiarity with electronic health record (EHR) systems and data management tools.
Responsibilities:
- Conduct comprehensive assessments of patients’ medical, psychosocial, and environmental needs to develop personalized care plans.
- Facilitate discharge plans to appropriate post acute care settings.
- Coordinate and communicate with physicians, nurses, social workers, and other health care professionals to implement and adjust care plans as needed.
- Monitor patient progress and outcomes, ensuring adherence to treatment plans and facilitating transitions of care between settings.
- Advocate for patients by identifying barriers to care and connecting them with appropriate community resources and support services.
- Maintain accurate and timely documentation of case management activities in compliance with regulatory and organizational standards.
- Educate patients and families about diagnoses, treatment options, and available resources to empower informed decision-making.
- Participate in interdisciplinary team meetings to discuss patient care plans and contribute to quality improvement initiatives.

The required skills enable the Case Manager to effectively assess patient needs, develop and implement care plans, and coordinate multidisciplinary care teams to optimize patient outcomes. Strong communication skills are essential for educating patients and collaborating with health care providers and community resources. Organizational skills support the management of multiple cases simultaneously while ensuring compliance with documentation and regulatory requirements. Critical thinking and problem-solving abilities allow the Case Manager to identify barriers to care and develop innovative solutions to improve patient access and satisfaction. Preferred skills such as proficiency with EHR systems and case management certification enhance efficiency and credibility, supporting continuous quality improvement and professional growth.
Monday through Friday; Rotating weekend coverage
40 hours a week; 8 hours shifts
