What Registered Nurse contributes to Cardinal Health

Nursing is responsible for the delivery of safe and efficient quality nursing care. Examines patients and administers prescribed medications to facilitate healing and increase comfort. Nursing requires training and or licensed to care for patients.

This Registered Nurse (RN) will be working at OncoLogics in Lafayette, LA. They are responsible for delivering direct patient care, collaborating closely with physicians and other healthcare providers to care for patients, including those diagnosed with cancer.

Responsibilities

Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements.
Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following company and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, Oncology Nursing Society and other governing agency regulations.
Resolves patient problems and needs by utilizing multidisciplinary team strategies.
Maintains safe and clean working environment by complying with policies, procedures, rules, and regulations; calling for assistance from health care support personnel when necessary.
Protects patients and employees by adhering to infection-control policies and protocols, safe medication administration, mixing, and storage procedures, and controlled substance regulations.
Documents patient care services by charting in patient and department records.
Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs.
Maintains patient confidence and protects operations by keeping information confidential.
Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques.
Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.
Updates and maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
Maintains a respectful cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.

Qualifications

Ability to work a flexible schedule as necessary
Associate’s Degree of Science in related field
Bachelor's degree in related field, or equivalent work experience, preferred
Active RN license for the state of Louisiana in good standing
Experience working with oncology patients, preferred
1 year of experience, preferred

Perinatal Nurse Navigator

The Perinatal Nurse Navigator is a nurse based in hospitals or clinics who provides targeted, time-limited support for pregnant and postpartum patients, particularly those with or at risk for substance use disorder (SUD/OUD) during critical transitions in their care. This role facilitates connections from the inpatient or birthing hospital to prenatal, postpartum, and community resources. Additionally, the Navigator works closely with the Project M.O.M. Regional Navigator to provide additional support for these patients. Essential Duties & Responsibilities: -Ensure safe and timely transitions between birthing hospitals, OB triage, emergency departments, and outpatient prenatal and postpartum care. -Identify and support pregnant and postpartum patients who are dealing with SUD/OUD -Connect patients to Project M.O.M. resources, providers capable of managing MOUD, and various socioeconomic supports. -Collaborate with hospital teams, including Labor and Delivery (L&D), Mother/Baby, Neonatal Intensive Care Unit (NICU), and Emergency Department (ED), as well as clinics and community partners, in line with the LDH Project M.O.M. initiatives. -Proactively identify pregnant and postpartum patients who are at risk for SUD & OUD through consultations, screenings, and referrals. Coordinate navigation for the following services: -Prenatal and postpartum care -Primary care -SUD/OUD treatment (including medication-assisted treatment providers) -Behavioral health and social services -Perform warm handoffs: -Hospital to Outpatient Clinic -Hospital/Clinic to Project M.O.M. Regional Navigator for ongoing support Prior to discharge: -Arrange prenatal and postpartum appointments and confirm scheduling in the Electronic Health Record (EHR) when applicable. -Conduct brief, time-limited follow-ups to: -Reinforce discharge instructions and treatment plans. -Confirm attendance or address missed visits. -Identify and address immediate barriers (transportation, insurance, childcare, stigma, etc.) -Provide targeted health education on topics such as pregnancy, postpartum warning signs, medication for opioid use disorder (MOUD), safe breastfeeding, safe sleep practices, overdose prevention, and access to naloxone. Promote stigma-free, evidenced based, trauma-informed care: -Educate labor and delivery (L&D), nursery, emergency department (ED), and clinic staff on: implementing consistent trauma-informed SU screening protocols, education on evidence-based approaches to substance use treatment including MOUD and harm reduction, respectful communication and bias reduction Documentation: -Document all navigation encounters, care transitions, and handoffs, and communicate updates to the care team. Education: Minimum associate's degree in nursing required Certification/License: Louisiana RN license and CPR required. Additional Experience Preferred -Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN), with a preference for RN candidates, who has experience in maternal-child health, perinatal care, or the NICU/OB environment. -The candidate should demonstrate skills in care coordination, transitions of care, and patient education. Training or a willingness to be trained in the following areas is required: -SUD and OUD in pregnancy -Motivational interviewing techniques -Trauma-informed and stigma-reducing communication -Local Project M.O.M. workflows and referral pathways -The role involves working across hospital and clinics to manage short-term navigation during delivery and the immediate postpartum period. -Key responsibilities include closely coordinating with the Project M.O.M. Regional Navigator and Ally Peer Navigators to initiate warm handoffs and coordinated care for person-centered, ongoing, long-term support.
