Summary

The Clinical Nurse Manager provides strong leadership and oversight for clinical services, programs, and staff, ensuring the delivery of high-quality patient care. This role is responsible for managing day-to-day clinical operations while fostering a collaborative, patient- and family-centered care environment.

Key Responsibilities

  • Lead and oversee clinical services and programs to ensure safe, effective, and evidence-based care
  • Provide direct supervision and support to a multidisciplinary team, including:
    • Registered Nurses (RNs)
    • Licensed Practical Nurses (LPNs)
    • Physical Therapists (PTs) and Physical Therapy Assistants (PTAs)
    • Occupational Therapists (OTs) and Occupational Therapy Assistants (OTAs)
    • Speech-Language Pathologists
    • Social Workers
    • Home Health Aides
  • Guide and support staff using the Quadruple Aim framework to promote quality outcomes, patient and family satisfaction, staff engagement, and efficient care delivery
  • Foster a highly reliable, collaborative, interprofessional care environment
This position reports directly to the Clinical Director.

Required Minimum Knowledge, Skills, And Abilities (KSAs)

  • Education: Bachelor’s Degree in Nursing required; Master’s degree preferred.
  • License/Certifications: Active, unencumbered Registered Nurse license to practice in the State of Maine. Current certification in Basic Life Support for Healthcare providers. Clinical specialty certification, highly desirable OASIS certification, preferred.
  • Experience: Three years of clinical experience; supervisory and home health management experience preferred
  • Strong knowledge of OASIS required.
  • Proficient with Electronic Health Record, OASIS, and PDGM.
  • Working knowledge regarding content, interpretation and implementation of agency policies and procedures.
  • Working knowledge of Microsoft Word, Excel, and Outlook
  • Ability to manage in an inter professional team to achieve Agency goals and quality care outcomes.
  • Working knowledge of the Medicare Conditions of Participation for Home Care and Maine State home health licensing regulations.

Manager of Care Coordination and Planning (RN)

Description Maine Medical Center Management/Leadership Req #: 65334 Maine Medical Center – Portland is seeking a Manager of Care Coordination and Discharge Planning (RN). This individual is responsible for the daily planning, assessing, monitoring and managing of the patient continuum of care. In partnership with other Care Management staff and physician leadership, to ensure all patients admitted to Maine Medical Center have a length of stay consistent with or less than medical and/or surgical criteria geometric mean length of stay. The Manager of Care Coordination and Discharge Planning (RN) serves as a resource for the care management team and other departments, and reports to the Director of Care Management. This position contributes toward the overall development of programs and services to provide for the needs of patients/families, physicians and staff. He/she assists the Director in providing strategic leadership to ensure equitable distribution of resources and delivery of high quality, cost-effective healthcare and efficient management of Care Management staff in accordance with goals, objectives, policies, and applicable regulatory agency rules and regulations. The Manager of Care Coordination and Discharge Planning (RN) ensures a strong focus on discharge planning beginning on the day of admission, to meet the needs of the patient/family as much as possible while remaining in compliance with federal, state and local regulations. This manager will be under the direct supervision of the Director of Care Management, and under the indirect supervision of the Chief Financial Officer. This is a Monday-Friday position and will sit onsite at the Bramhall Campus. Required Minimum Knowledge, Skills, And Abilities (KSAs) Education: Bachelor's degree in Nursing or Master's degree in SW.
