$41.10 - $61.65EDUCATION/EXPERIENCE:
- Graduation from an accredited School of Nursing with a bachelor's degree in nursing required.
LICENSURE, CERTIFICATION, and/or REGISTRATION:
- Current license to practice as a Registered Nurse (RN) in the State of applicable state required.
- CCTC preferred.
ESSENTIAL FUNCTIONS:
- Adheres to the general hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.
- Ensures the maintenance of the pre transplant charts, ensures compliance with documentation required by regulatory agencies. Documents the patient's plan of care and assures documentation that demonstrates the interdisciplinary nature of pre transplant patient care. Consistently uses the transplant navigator in WakeOne and ensures that it is updated as the patient moves through the phases of care. Works toward the reduction of paper documentation so that the EMR is complete and available to all caregivers in any setting.
- Coordinates pre-transplant patient education and evaluation processes. Presents patients to Selection Committee for consideration of listing for transplant.
- Assumes responsibility for call. Demonstrates the ability to accurately and timely provide information about listed patients and coordinate a transplant in collaboration and at the direction of the attending surgeon.
- Serves as a liaison with regional dialysis centers and nephrology offices for patient referrals, education, and pre transplant follow up processes. Regularly communicates with them regarding patient status during the evaluation phase.
- Maintains exemplary customer service skills by assisting customers, responding to questions, problem solving, promptly answering telephone in accordance with hospital and departmental standards, and maintaining a professional image. Demonstrates positive interpersonal relationships with co-workers and physicians.
- Engages in self-development to maintain professional and technical knowledge base and professional credentials.
- Ensures age/developmentally appropriate patient care is provided in accordance with Age specific Care Guidelines for the age group served.

Coordinates the activities relating to recipient evaluation, pre transplant education, and case management of pre transplant recipients within the Abdominal Organ Transplant Program. Promotes patient and family satisfaction and effective and efficient care through the referral and evaluation process. Coordinates the evaluation with all members of the interdisciplinary team and serves as a liaison with dialysis centers and referring physicians related to the care of patients referred to the program and in the evaluation phase of care.
