The Manager, Case Management oversees the day-to-day operations of the point of entry for the Patient Case Management department and works collaboratively with departmental leadership and the physician advisor to carry out department goals. Implements and maintains daily departmental operations with consistent standards of practice. Maintains clinical competency and serves as clinical resource to staff of the department, physicians, and nursing leadership to support patient progression and appropriate resource utilization throughout the system. Collaborates with physicians, nursing, members of the interdisciplinary team, Patient Financial Services, Patient Access and other departments to ensure that quality services are being provided timely and serves as a resource to resolving barriers. Analyzes data to improve the effectiveness of the Patient Case Management program.
