Responsibilities:

  • Provide high-quality patient care and support to clients in their homes, following established care plans and protocols

  • Assist patients with daily living activities, such as bathing, dressing, and grooming

  • Monitor patient vital signs and report any changes or concerns to the nursing team

  • Assist with meal preparation and management, as needed

  • Maintain a clean and safe living environment for patients

  • Communicate effectively with patients, families, and healthcare professionals to ensure comprehensive care

  • Participate in ongoing training and professional development to stay up-to-date on best practices and industry standards

Requirements:

  • Certified Home Health Aide (CHHA) certification

  • 1 year of experience in home health care or a related field

  • High school diploma or equivalent required

  • Ability to lift and transfer patients with proper body mechanics and equipment

  • Ability to work varying shifts, including days, evenings, and weekends

  • Excellent communication and interpersonal skills

  • Current CPR certification
