Job Summary:

We are seeking a compassionate and dedicated Activities Assistant to join our team at Terraces of Lake Worth Care Center, a premier skilled nursing center located in Lake Worth, Florida. As an Activities Assistant, you will play a vital role in creating a vibrant and engaging community for our residents, promoting their physical, emotional, and social well-being. This is a part-time position, offering flexible weekend hours.

Responsibilities:

Plan and implement activities that promote socialization, recreation, and fulfillment among residents, including arts and crafts, games, exercise programs, and entertainment events
Develop and maintain relationships with residents, ensuring their needs and preferences are met
Collaborate with interdisciplinary teams to identify and address resident care needs and concerns
Conduct assessments to identify resident interests and abilities, and develop individualized activity plans
Maintain accurate records and reports of activities, including participation rates and resident feedback
Assist with resident transportation to off-site activities and appointments
Participate in staff meetings and professional development opportunities to stay current on best practices and industry trends

Requirements:

High school diploma or equivalent required
CNA license
1 years of experience in the senior care industry or related field
Ability to work flexible weekend schedules
Excellent communication, organizational, and interpersonal skills
Ability to work independently and as part of a team
Commitment to providing exceptional customer service and promoting a positive work environment

If assigned to a psychiatric patient, aide/CNA has documented experience in additional training to be able to meet the needs of this specialized population. Duties of the Home Health Aide/CNA include, but may not be limited to: • Provides assistance with personal care, hygiene, and activities of daily living. • Encourages client participation in activities to the extent to which the client is able. • Assists with ambulation as instructed. • Assists client to: a) bed, b) commode, c) chair. • Turns and positions bed bound clients. • Measures and records intake/output, as assigned. • Measures and records temperature, pulse and respiration on each visit. • Changes bed linen if needed. • Prepares simple meals following dietary instructions as instructed. • Maintains a neat and clean environment. • May grocery shop one time a week for list of ten items or less as needed. • Informs supervisor of any changes in client’s condition or home situation. • Follows care plan as written. • Provides documentation of care given on Agency approved forms. • Reports any changes in patient’s condition, living conditions etc., to RN/Supervisor as they occur. • Performs any other task/duty that is specifically assigned by supervisor, and for which aide has been specifically trained.
