Serenity Hospice Care is looking for compassionate and dedicated Licensed Practical Nurses to join our team in  New Jersey. This individual will be responsible for care & services to patients as needed. Candidates must hold a valid LPN License in the state of New Jersey in order to be considered.

This position will be 7PM to 8AM and will be 1:1 for patients. 

Responsibilities:

  • Care for patients in the location as needed.
  • Assist with activities of daily living.
  • Follow care plans and deliver the highest level of care.
  • Document medical information as required. 
  • Collaborate with interdisciplinary team.
  • Communicate with families and patients.

Requirements:

  • Valid LPN License in NJ.
  • Prior Hospice Experience a Plus!
  • Drivers License with Auto Insurance. 
  • Reliable Transportation.

School LPN Nurse Needed in the Bronx 2 days per week, 9am-5pm! Scope of Services Consultant will coordinate and oversee the health care services across the 2 sites. This includes: -Completing all screenings including vision, height, dental, weight and lead. Dental and vision will be monitored and charted if completed prior by the physician. Weight will be monitored and recorded if completed prior and nurse will do follow up after 3 months. This also includes managing all health and nutrition components of the programs in accordance with federal and state regulations, implementing the medication policy including administration of medication, measuring blood pressure, blood sugar and monitor records to ensure compliance with regulatory standards, and function as part of a cohesive team that develops and monitors program operations.
