Position Summary

As an LPN at Charter Senior Living, you are a key clinical leader and advocate for our residents. You will support residents in Assisted Living and/or Memory Care by delivering high-quality, compassionate nursing care that promotes safety, dignity, and quality of life.

Key Responsibilities

Conduct resident assessments and assist with individualized care planning
Administer medications and treatments safely and accurately
Monitor resident conditions and communicate changes promptly
Document nursing services and resident responses
Collaborate closely with caregivers, leadership, and care teams
Support residents with patience, professionalism, and a positive attitude

Qualifications

Active LPN license in the state of Tennessee
Assisted Living and/or Memory Care experience preferred
Strong clinical judgment and attention to detail
Compassionate, team-oriented approach to care
Commitment to resident-centered service

Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes. Teach patients and caregivers self-care techniques.
