Key Responsibilities:

  • Participate in developing comprehensive, individualized care plans for each resident

  • Ensure resident care plans are completed, updated, and reviewed in accordance with RAI guidelines and changes in resident needs

  • Manage and implement the Clinical and Prospective Payment System (PPS) MDS completion schedule

  • Direct, organize, and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care, and HMO reimbursement

  • Coordinate and oversee all aspects of the MDS assessment process to ensure compliance with federal and state regulations

  • Participate in pre-admission evaluations to determine potential residents’ level of care

  • Assist with level-of-care determinations upon admission and monitor changes requiring timely MDS assessments

  • Review MDS assessments and related documentation to ensure accuracy and timely completion

  • Monitor reports related to MDS and care plan compliance and follow up as needed

  • Analyze Quality Indicator reports and ensure findings are reflected in resident care plans

  • Maintain current knowledge of state and federal regulations related to RAI, care planning, and PPS

Qualifications:

  • High school diploma or equivalent

  • Nursing experience preferred

  • RN preferred; LPN candidates encouraged to apply
