We are currently seeking an experienced Registered Nurse to join our team as a Home Infusion Coordinator. In this role, the Coordinator will support patients from Rex, Rex Holly Springs, Nash and UNC Johnston Hospitals, collaborating with clinical case managers and the multidisciplinary team to initiate and coordinate home infusion services with our post -acute care team.

The ideal candidate will have a minimum of two years of nursing experience, with a strong foundation in Medical-Surgical nursing and a working knowledge of infusion therapy.

We are seeking RNs who:

  • Are proactive and demonstrate strong communication, critical thinking, and time management skills
  • Have experience in Home Health and/or Home Infusion
  • Are skilled in delivering individualized care tailored to each patient’s needs and home environment
  • Are able and willing to travel between UNC hospitals

This full-time, state-employed position is scheduled Monday through Friday from 8:30 a.m. to 5:00 p.m., with some on-call responsibilities. The role is eligible for comprehensive full-time benefits and a generous paid time off accrual program.

Summary:

Provides patient care coordination for home health or hospice patients. Educates physicians and staff and facilitates continuity of care of patients from hospital to home or hospice. Participates on interdisciplinary team and collaborates with physicians and staff to establish and implement home care or hospice needs. Coordinates referrals from skilled nursing facilities or other locations.


Responsibilities:
1. Communicates all appropriate patient information and coordinates staff availability directly with the home health or hospice intake department. Communicates home health or hospice staffing availability to referral sources and staff.
2. Coordinates with the patient, patient’s family, referring physician, clinical care management representative, and hospital staff to determine the appropriateness of all home care referrals.
3. Educates referral sources and other hospital staff in relation to home health or hospice services and regulatory requirements.
4. Gathers all the necessary demographic and medical information, as well as all requested treatment and services in order to develop a detailed and comprehensive home health or hospice care referral and plan. Identify the type of service which best serves the referral patient: General Inpatient Hospice or Home Hospice and communicate this to the referral source (Case Managers, Residents, or Physicians) and patient/family/care giver.

● If hired after October 1, 2015 must be enrolled in an accredited program within four years of employment, and obtain a Bachelor's degree with a major in Nursing or a Master's degree with a major in Nursing within seven years of employment date.
