Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
Records pain/symptom management changes/outcomes as appropriate.
Teaches patients, families, and caregivers appropriate patient care techniques.
Regularly communicates with clinical manager and care team.
Performs on-call responsibilities and on-call services to patients/families as assigned.
Other duties as assigned.

Qualifications

One year of nursing experience.
Current RN license, specific to the state(s) you are assigned to work.
Current CPR certification.
Valid driver's license, reliable transportation and liability insurance.

Promotes and educates about VNA programs and services. What You’ll Do: 1. Increases volume of referrals by building long-term relationships and ensuring excellent customer service. 2. Makes regular site visits to facilitate referral coordination. 3. Identifies problems and adverse referral situations; and works collaboratively with referral sources and agency staff to correct same. Takes a lead role in any customer service and/or quality improvement efforts derived from customer satisfaction surveys or client concerns. 4. Maintains comprehensive working knowledge of coverage guidelines for VNA services and provides education to staff and physicians as needed. 5. Maintains comprehensive working knowledge of the VNA’s markets including major payer groups, key referral sources and competitors market positioning. 6. Collaborates with hospital staff & VNA staff to effectively transition patients to VNA Manchester, and monitor readmissions. Who You Are: . Education : Graduate of an accredited school of nursing, education in Sales and Marketing or related field required. Experience : Three (3) to five (5) years experience in sales and marketing, homecare, or related required. Certification/Licensure: Current Licensure in State of New Hampshire as a professional nurse preferred. Software/Hardware : Familiar with MS Outlook, Work, Excel and/or Access.
