Liaises between Care Management and Hospitalist providers to address patient challenges.
Communicates daily with Hospitalist providers to review and update patient discharge plans.
Participates in interdisciplinary meetings to discuss patient care and discharge strategies.
Communicates information with providers and facilitates changes in plan of care as directed by physician.
Acts as an escalation point for complex cases and issues.
