  • Receive transferred patient calls for clinical triage from call center staff
  • Perform comprehensive symptom assessments via telephone
  • Utilize established triage protocols to determine level of urgency and disposition
  • Identify red-flag symptoms and initiate escalation to provider or emergency services as appropriate
  • Work closely with medical assistants, providers, and front desk staff
  • Assist in coordinating urgent add‑on visits
  • Route patients appropriately (same-day visit, future appointment, urgent care, ED, or self-care)
  • Document all assessments, advice, and dispositions accurately in the EHR
  • Document assessments and triage decisions accurately in the EHR
  • Maintain patient safety, confidentiality, and professional boundaries in all interactions
  • Collaborate with call center staff to ensure smooth call transfers and accurate patient information
  • Manage high call volumes while maintaining clinical accuracy and professionalism
  • Prioritize multiple calls based on clinical urgency
  • Participate in quality improvement efforts related to access, triage accuracy, and patient satisfaction
  • Office based duties as required
  • Active RN license (state‑specific requirements apply)
  • Minimum 3 years of clinical experience (primary care, urgent care, or triage preferred)
  • Strong assessment and critical‑thinking skills
  • Excellent communication and multitasking abilities
  • Comfortable working in a visible, patient‑facing role
  • Proficiency with Athena electronic health records preferred
