The Case Manager Registered Nurse interacts with patients, physicians, and other members of the healthcare team to assure appropriateness of patient placement and education.
They will provide outpatient and telephonic education, monitoring, and appropriate interventions to identify high risk inpatients.
They will encourage appropriate utilization of services.
They will promote timely discharge.
They will provide education and in-services to members of the healthcare team when necessary.
They will prepare and furnish reports to hospital committees and leadership.
They will participate in problem solving activities
They will perform other duties as assigned.
