Overview
The Care Coordinator, RN is a Registered Nurse who serves as an advocate and shares a commitment to excellence, innovation and compassionate care for patients with advanced heart failure and pulmonary hypertension. The Care Coordinator, RN will collaborate with an interdisciplinary team to navigate the plan of care and facilitate the coordination of services across the health system. work well on a team and collaborate closely with patients and team members.
