Your Role:

Provide compassionate, high-quality care to residents in our skilled nursing facility
Administer medications and treatments as prescribed and within your scope of practice
Collaborate with a team to ensure coordinated care and optimal outcomes for residents
Monitor patient conditions, including vital signs, and report changes to the Director of Nursing
Promote restorative care to encourage residents’ independence and well-being
Maintain accurate and thorough documentation of all nursing care provided
Support and mentor nursing staff to foster a positive, patient-centered environment
Ensure patient comfort, safety, and well-being while adhering to facility standards

About the position: This position is responsible for providing direct and indirect patient care in the department setting. Communicates with physicians/Nurse Manager/co-workers, as appropriate about changes in patient’s clinical condition including abnormal assessment findings, results of diagnostic studies, and symptomatology. Responds quickly and accurately to changes in patient condition or response to treatment. Additionally, is able to perform general nursing duties in all departments with adequate supervision. Utilizes independent judgment regarding priority of work, changes in patient condition, evaluating patient's response to care and emergency situations. Strives for excellence in clinical practice, patient/family teaching and communication skills. Participates in performance improvement activities of the department and daily management of infection prevention and control activities. This is a Night Shift 7PM - 7AM position! Primary Functions: Performs age related patient assessments and reassessments as per department policy. This includes neonates, pediatric, geriatric and the general patient population. Able to assess and reassess pain. Utilizes appropriate pain management techniques. Recognizes, interprets, documents and communicates significant information (Situation, Background, Assessment, and Recommendation) necessary for quality patient care and related patient information. Collects relevant patient history data. Documents patient assessment, clinical interventions and responses following current policies and procedure Collaborates with the patient, family and other health team members to provide humane, caring nursing care that promotes healing. Administers medications and treatments as ordered following the five medication rights. Reduces the potential for errors. Identifies individualized, expected outcomes for the patient. Creates plans of care that prescribe interventions to attain expected outcomes and that are appropriate to the age and development level of the patient. Formulates a teaching plan based on the plan of care. Educates the patient and family regarding the plan of care, expected outcomes, and identified learning needs. Maintain knowledge of the laws and rules regulating nursing. Perform nursing practice within the legal scope of nursing practice to the level of licensure. Treats patients and significant others with dignity and respect, ensures confidentiality of patient records. Interacts professionally with patients and significant others, providing explanations when necessary. Other Duties As Assigned: Accepts other duties/responsibilities as assigned. Minimum Qualifications: Education: Current Registered Nurse licensure in State of Ohio required BLS certification within 30 days of employment required. ACLS certification within 6 months of employment required. PALS certification within 6 months of employment required. CEN, NRP, TNCC certifications preferred. Experience: Experience in an Emergency or Critical Care setting is preferred Knowledge, Skills and Abilities: Successful completion of appropriate competencies for assigned specialty areas.
