Job Description

Hospice Aides provide personal care and designated health related services in compliance with policies and procedures, applicable health care standards, and government laws and regulations.

Responsibilities

Assists in the implementation of the patient Plan of Care., Assists patients with activities of daily living., Attends in-services, staff meetings, and continuing education., Follows verbal and written directions provided by the RN, specific to the patient assigned., Maintains a clean and safe environment and performs light housekeeping assignments as assigned., Maintains and submits documentation as required. Records observations, completed, and refused tasks., Observes, records, and reports significant changes in the status of a patient and/or family to supervisor., Participates in interdisciplinary team meetings., Prepares light meals if assigned in patient plan of care., Provides hands-on personal care to patients., Provides personal care and supportive services to hospice patients.
