Position Summary
The Licensed Practical Nurse (LPN) plays a vital role in providing clinical nursing care to residents in our Assisted Living and Memory Care programs. This position supports resident assessments, care planning, medication administration, and ongoing monitoring while promoting dignity, comfort, and quality of life.
LPNs at Charter collaborate closely with caregivers, CNAs, families, and leadership to ensure safe, person-centered care.
Key Responsibilities
Provide hands-on LPN nursing care in an assisted living and memory care setting
Administer medications and treatments in accordance with physician orders and state regulations
Assist with resident assessments, care plans, and health monitoring
Document nursing services accurately and timely
Respond to changes in resident condition and communicate with leadership and families
Support and collaborate with caregivers and care staff
Promote resident safety, dignity, and independence
Uphold Charter’s mission, values, and quality standards
Qualifications
Active Massachusetts LPN license (required)
Assisted Living, Memory Care, or long-term care experience preferred
Strong medication management and clinical documentation skills
Compassionate, dependable, and team-oriented approach
Passion for serving seniors and making a meaningful impact
