Serenity Hospice is looking to add a dedicated and compassionate Admissions Nurse to delivering exceptional care, comfort and support to patients. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. This role will oversee the Burlington/Mercer County areas.

The position will work the following schedule:

Monday-Friday 3P-11P

Responsibilities:

Conduct timely, comprehensive assessments to determine hospice eligibility based on clinical criteria and Medicare guidelines.
Clearly and compassionately explain hospice philosophy, services, and benefits to patients and families.
Initiate the admission process and obtain all necessary consents, documentation, and physician orders.
Coordinate with the hospice interdisciplinary team to ensure seamless onboarding and transition of care.
Provide patient and family education on symptom management, medications, and what to expect during hospice care.
Collaborate with hospitals, nursing homes, physicians, and referral sources to facilitate smooth transitions into hospice.
Accurately document all assessments, findings, and communications in the electronic medical record (EMR) in a timely manner.
Maintain compliance with federal, state, and organizational policies and procedures.

Requirements:

Current NJ RN License in good standing
Minimum 1 Year Hospice Experience REQUIRED
Willing to travel to the assigned areas
Must have reliable transportation with liability insurance
Strong communication and interpersonal skills.
