We have an exciting opportunity for a full-time Licensed Practical Nurse (LPN) to support our Spravato program in Livewell. The Licensed Practical Nurse (LPN) is responsible for providing direct patient care and support to our patients in our Outpatient Mental Health Counseling Center: to include our Spravato (Esketamine) program and Outpatient Substance Use Disorder services. Spravato (Esketamine) is an innovative treatment that can provide relief after only 24 hours.This role requires the LPN to assist in patient care, monitor vital signs, provide support during treatment sessions and ensure adherence to safety protocols. Additionally, the LPN will collaborate with the multidisciplinary healthcare team to develop and implement individualized care plans aimed at optimizing patient outcomes and facilitating a supportive environment for those receiving a wide range of mental health services. The LPN will perform these duties collaboratively and under the supervision of registered nurses, advanced practice providers and physicians. Additionally, the LPN is responsible for educating patients regarding their treatments, treatment options and ensuring a therapeutic and safe environment throughout their mental health treatment journey. The Licensed Practical Nurse must also maintain accurate medical records, communicate and collaborate effectively with our multidisciplinary team, and ensure adherence to safety and all Organizational, State and Federal regulatory standards. Professional and effective communication and collaboration with our healthcare team is essential to optimize patient outcomes in this specialized setting. Competencies:
- Proficiency in providing care for patients diagnosed with a Mental Health illness is essential.
- Adheres to all REMS protocols for Esketamine.
- Signs and dates all entries made in the patient's medical record.
- Charts nurses' notes in an informative and descriptive manner that reflects the care provided to the patient as well as the ' response to the care.
- Prepares and administers medications as ordered by the physician and within the guidelines of good nursing practice.
- Ensures that discontinued drugs and narcotics are turned in to the Nursing Supervisor/Unit Manager for destruction or returned to provider pharmacy for credit.
- Reviews medication orders for completeness of information, accuracy in the transcription of the physician's order, and adherence to stop policies.
- Makes daily rounds of assigned patients in accordance with acceptable nursing standards.
- Transcribes physician's orders/LP orders, as per policy.
- Supports patients’ well-being through prevention and education, providing health promotion and maintenance.
- Conducts intake screening as needed, evaluation and triage.
- Proficient in understanding Mental Health diagnosis/s and nursing care planning of the same.
- Reports all discrepancies noted concerning physician's orders, charting error, to the Director of Nursing or Supervising Provider.
- Fills out and completes Transfer Forms in accordance with established procedures.
- The individual is competent to care for adult and geriatric (ages 65+)
- Complete all documentation as required.
- Participates in the maintenance and implementation of the facility's quality assurance program for the nursing services department, e. Restraint reduction.
- Interprets the department's policies and procedures to personnel, patients, visitors and government agencies as required.
- Admits, transfers, and discharges patients as Fills out and completes accident/incident reports. Submit to Nurse Manager as required.
- Charts all accidents/incidents involving the patients following established procedures.
- Takes and records TPR's, blood pressures, as necessary.
- Participates in the implementation and ensures maintenance of the infection control program for monitoring communicable and/or infectious diseases among the patients and personnel i.e. universal precautions guidelines.
- Maintains patient rights and confidentiality of all patient care information.
- Monitors nursing care to ensure that all patients are treated fairly and with kindness, dignity, respect and privacy.
- Reviews complaints and grievances made by the patient and makes a written/oral report to the Nurse Supervisor/Unit Manager indicating what action(s) were taken to resolve the complaint or grievance.
- Ensures that assigned nursing personnel follow established departmental policies and procedures including dress code and safety regulations.
- Reviews patient care plans for target goals and individualized interventions, problems, approaches, and revisions based on nursing needs. Informs the Director of Nursing or Supervising Provider of any changes that need to be made to the care plan. Ensures that nurses' notes reflect that the care plan is being followed.
- Participates in implementation and maintenance of the procedures for reporting hazardous conditions or equipment.
- Orders prescribed medications, supplies and equipment as necessary and in accordance with established policies.
- Ensures that an adequate supply of medications, supplies and equipment are on hand to meet the nursing needs of the patients. Report needs to the Director of Nursing or Supervising Provider.
- Ensures that narcotic records are accurate for shift, for those patients Notifies the Nursing Supervisor/Unit Manager of all drug and narcotic discrepancies noted on your shift.
- Ensures that the written policies and procedures that govern the day-to-day functions of the nursing services department are followed by all nursing personnel assigned.
- Reports problem areas to the Director of Nursing or Supervising Provider. Reviews complaints and grievances made or filed by assigned personnel. Makes appropriate reports to the Nurse Supervisor/Unit Manager.
- Ensures that department personnel, patients and visitors follow the department established policies and procedures at all times.
- Provides leadership to nursing personnel assigned to unit/shift. Creates and maintains an atmosphere of warmth, personal interest and positive emphasis as well as a calm environment throughout the unit and shift.
Schedule: 9am-5pm, Monday- Friday.
Education: A graduate of an accredited school of nursing.

QUALIFICATIONS/REQUIREMENTS
Demonstrates compliance with the CH ASPIRE Standards of Performance.
Qualifications:
- Previous experience in a mental health or related field preferred.
- Must possess a current, unencumbered license to practice as a LPN in New Jersey.
- Training in de-escalation techniques and intervention required within 1 month of employment.
