Stroke Program - Nurse Navigator - Full Time - Days

Job Summary

The Stroke Nurse Navigator is responsible for deploying tactics developed by the leadership team to assist in meeting program goals and objectives of the program. Systematically evaluates the care delivery from pre, intra, and post hospital care (where applicable) to identify opportunities for process improvement and assist relevant process owners with their process improvement.  Collaborate with the stroke program coordinator and the outreach nurse to plan and implement materials and programs to support post stroke survivors and their caregivers.   
All Nurse Navigators act as the primary point of contact and is responsible for monitoring, coordinating care and educating patients and families.  Offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial through all phases of the healthcare continuum.

Core Job Responsibilities

  • Serves as a resource nurse expert to staff to assist in identifying barriers, locating resources, coordinating services, providing education on stroke; refer patients to integrative therapies and support groups. Applies clinical expertise and nursing experience to concurrently and/or retrospectively analyze patient care delivery documentation from all available resources to evaluate process timeliness and effectiveness, education needs, compliance issues or question of clinical judgement.
  • Serves as primary contact and advocate for patient; builds and maintains trust of patient and family; provide emotional support; navigate the health care system using knowledge of the intricacies of each system and serving as the patient advocate assuring timely access; from ED arrival to post discharge follow up; serve as point person for physicians.
  • Responds to all Code Stroke (inpatient and ED) when available.  Assists the team in clinical care, documentation and process support.  Facilitates communication with the Stroke Team and other functions as required.
  • Sees every newly hospitalized stroke- related diagnosis patient.  Provides welcome package and reviews content.  Explain Stroke Team Member roles, describes what to expect during hospitalization.  Participates in Team Rounds.  Documents in patient’s record when appropriate.  Reviews active patient charts regularly.
  • Sees all hospitalized stroke- related diagnosed patients prior to discharge.  Reinforces plan of care, emphasizes medication compliance and follow up appointments.
  • Serves as a consultant throughout stroke diagnosis and recovery.
  • Maintains/utilizes a comprehensive database of resources, both locally, regionally, and nationally, for patients and families including financial, psychosocial, education, emotional, transportation, and cultural support.
  • Collaborates with multidisciplinary team and bring process issues to stroke leadership team.
  • Creates and maintains open lines of communication with treating physicians, primary care provider, and their support staff; establish referral pathways thereby decreasing treatment delays: working collaboratively with the Neurocritical Care and Neuroendovascular to maintain a network of specialists and support of best practices in care delivery during code strokes and daily rounds; and to participate in stroke conferences.
  • Based upon care delivery observations, makes recommendations for initiatives to improve process and outcome performance and participates in support of those initiatives.  
  • Quality assurance; Maintains accurate and complete documentation; ongoing assessments of role in decreasing disparities in care and reduction in outmigration of patients; submits data to stroke coordinators for quality monitoring. 
  • Facilitates Stroke Support Group meetings monthly. Coordinates and lead inpatient and post discharge survivor and caregiver activities; leads efforts to continue improve the breadth and depth of services provided.
  • Conducts follow up calls (30, 90 day) and completes associate documentation.
  • Performs related duties as assigned.

Education/Experience Requirements

REQUIRED:

  • 5 years of experience in the area of specialty.
  • Problem solving, decision making and critical thinking.
  • Attention to detail, organized and skilled at follow up.
  • Excellent leadership, written, and verbal communication and interpersonal skills.
  • Ability to work in a self-directed environment.
  • Excellent presentation skills.
  • Highly proficient with MS Office and EMR.

 

Licensure/Certification Requirements

REQUIRED:

  • Active NYS RN licensure.
  • BLS and ACLS certification

PREFERRED:

  • BSN
