Be knowledgeable of residents’ DNR/DNI status, ensuring all non-hospital DNR forms accompany residents. Advocate proactively for residents during clinic visits, communicating any challenges back to the facility. Represent Nassau Rehabilitation and Nursing Center professionally at all times, adhering to policies, procedures, and abuse laws on and off facility property. May assist with additional clinic visits or unit staff duties as needed. Requirements Current New York State Home Health Aide (HHA) certification required. Ability to perform all tasks and duties of a Home Health Aide. Strong communication skills: ability to read, write, speak, and understand English. Knowledge of age-appropriate care for residents.
