Role Description This is a full-time on-site role for a RN Hospice Case Manager  in Edgewater, NJ. The RN Hospice Case Manager will be responsible for discharge planning, utilization management, nursing, medicine, and case management. Qualifications • Discharge Planning and Utilization Management skills • Nursing and Medicine knowledge • Experience in case management • Excellent communication and interpersonal skills • Ability to work in a team and independently • Bachelor's degree in Nursing or related field • Experience in hospice care is a plus • Current RN license in the state of New Jersey
