20. Reviews and revises plan of care and treatment plan according to patient’s status.

21. Completes patient classification for assigned patients as required.

22. Incorporates patient teaching/discharge planning to promote optimal patient outcomes.

23. Provides, evaluates retention of, and documents education to the patient/family/significant other according to organizational standards.

24. Coordinates and documents appropriate discharge planning and referrals to ensure continuity of care after discharge.

25. Provides pre-operative and post-operative teaching on all new surgical patient, if applicable.

26. Sees patients independently under Nurse Visit Only, those on infusion/active treatments, for pre-op surgical consent review, for post-op voiding trials, wound care and staple removal, if applicable.

27. Recognizes, accepts responsibility for, and maintains commitment to professional development and clinical competency.

28. Attends required professional development sessions and staff meetings. If unable to attend, review minutes or discuss proceedings of meetings with peers or managers.

29. Identifies personal knowledge deficits and seeks resources/consultation to ensure safe patient care.
