The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities:

  • Responsible for assisting patients with and per plan of care, the following:
    • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.
    • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
    • Safe transfer and ambulation techniques
    • Exercise according to the plan of care
    • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
    • Patient’s laundry
    • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
    • Observation and teaching of home safety practices
    • Recognition of an emergency situation and knowledge of emergency management procedures
  • Must follow patients’ plan of care and complete all tasks assigned
  • Communicates in a constant, clear, and effective way with the client, family/caregiver and office
  • Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements:

  • Meets all training requirements in accordance with state and federal laws when applicable
  • Ability to read, follow written instructions and document care given
  • Possess and maintains current CPR certification in accordance with state regulation
  • PPD or chest x-ray if applicable
  • Successful completion of Maxim screening examinations
  • Participation and completion of Maxim’s competency program

Must meet all federal, state, and local requirements

Maintains a safe, secure, and healthy patient environment by following asepsis standards and procedures, maintaining security precautions, and following prescribed dietary requirements and nutrition standards. Protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements. Enhances service reputation by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to job accomplishments. Use your skills to make an impact Required Qualifications Current State License as a Certified Nursing Assistant or Home Health Aide certificate completion of 75 hours CPR certification This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. This role is part of Humana's Driver safety program and, therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
