Responsibilities

  • Verify plan of care with the RN prior to each visit.

  • Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
  • Teach patients and caregivers self-care techniques.
  • Check vital signs.
  • Review all medications and perform a medication reconciliation at each visit.
  • Perform specialized procedures ordered by physician (i.e. foley catheter change).
  • Prepare equipment and materials for treatments using the aseptic technique.
  • Complete documentation timely and accurately.


Qualifications

  • One year of clinical LPN experience.
  • Current LPN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation and liability insurance.

Evaluates with the registered nurse the effectiveness of the LPN's nursing service to the patient and family under the guidance of the registered nurse. Maintains accurate and complete records of observations, treatments and care of patient. Prepares clinical and progress notes. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Assists the patient in learning appropriate self-care techniques. Participates in patient care conferences and discusses the need for involvement of other members of the health team, such as physical therapist, speech therapist, occupational therapist, and medical social worker. Submits any changes in schedule to clinical manager on a daily basis. Understands and adheres to established policies and procedures. Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive. ​ Minimum Requirements Education - Completion of an LPN program recognized by the SC State Board of Nursing. Experience - One (1) year experience as an LPN. Experience in Home Health preferred. In Lieu Of In lieu of one (1) year of LPN experience, will accept successful completion of the Prisma Health Nurse Residency Program or successful completion of a comparable residency program from another organization. In lieu of completion of a LPN program, will accept program equivalency recognized by the SC State Board of Nursing. In lieu of one (1) year of LPN experience, will accept candidates enrolled in an accredited LPN to RN transition program. ​ Required Certifications, Registrations, Licenses Licensed to practice as an LPN in South Carolina SC Driver's license. ​ Driving is required for this position using the employee's personally owned vehicle. Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies. If an LPN team member working in this position obtains RN licensure, Prisma Health will accept RN licensure for a limited time until the team member can be placed in an RN position.
