What You’ll Do:

Work with the interdisciplinary care team to ensure accurate patient assessment and care planning
Maintain open and ongoing communication with patients and families
Provide compassionate and appropriate nursing care
Assist with medications and treatments
Perform other patient care duties as assigned

What We’re Looking For:

Missouri Licensed Practical Nurse (LPN) license
Caring, compassionate, and positive attitude
Team player who works well in a collaborative environment
Prior Matrix and SNF experience is a plus
