The Advanced Practice Nurse (per diem) is part of the HIV Clinical Practice team and is responsible for ensuring that the total health needs of patients are met. This includes people living with HIV across the life span. Key job responsibilities include:
  1. Performs comprehensive physical and psychosocial assessments. Develops and implements plan for care in patients across the lifespan in collaboration with interdisciplinary team. Performs episodic acute care visits as needed.
  2. Orders patient diagnostic procedures and prescribes medication with her/his/their scope of practice and in accordance with the regulations of the institution and New Jersey Nurse Practice Act.
  3. Documents all healthcare contacts.
  4. Provides ongoing patient and family education related to health status, health maintenance, disease process and treatment options.
  5. Attends interdisciplinary patient care conferences.
