Contact Center Triage Nurse, GI Specialty - Part Time
The Contact Center GI Triage Nurse, triages patients telephonically to facilitate patients' access to the appropriate level of care by assessing needs, offering a clinical option, referral to health care providers, services, and community resources. Promotes wellness and informed decisions by providing health information, home care advice, and prevention concepts.
The nurse ensures timely access to appropriate care and seamless coordination of services throughout the continuum. Works collaboratively with physicians and other health team members, and is available to patients and families for care coordination.
Answers clinically related inbound calls. Maintains current knowledge base including triage skills, appropriate teaching principles,and policies and procedures.
Initiates all forms of communication for each encounter using accurate medical history and documentation, including patient education as well as reviewing test/procedure preps.
Acts as a patient advocate by resolving problems or communicating patient needs and concerns to appropriate resources.
Strong communication, interpersonal and problem solving skills to advocate for optimal patient outcomes.
Resource to assigned scheduling team responding to clinical questions as relates to prescriptions, appointments, medical management, practice protocols and appropriate population management.
Proficient in the use of multiple computer applications including EMR, spreadsheet programs and electronic medical records.
Responsible to gather information on diagnostic tests, previous hospitalizations and ambulatory care, leads communication to obtain an accurate medical history, fully understands reason for referral, or post hospital care.
Work Experience:
- Massachusetts license as an RN and BLS CPR certified.
- Minimum 5 years of nursing experience. Ideal candidate will experience in GI, Endoscopy, colonoscopy procedure, pre admission testing
- Patient education and triage in GI Specialty is essential and will help this nurse be successful in this dynamic, autonomous role
- Excellent communication, interpersonal and problem solving skills to advocate for optimal patient outcomes. Telephonic triage strongly preferred.
- Proficient in the use of multiple computer applications including EMR, spreadsheet programs and electronic medical records.
- Exceptional communication and computer skills
- Must have competency in delivering exceptional nursing care and have proven success in partnering with both physicians and staff
Other Information
Ability to work remote after successful completion of onsite orientation and training. Must be available to work occasionally on site as needed
