Be the Best Home Health LVN/LPN You Can Be

If you meet these qualifications, we want to meet you!

One year of clinical experience and one year of home health experience
Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records

Required Certifications and Licensures:

Graduate of an accredited practical or vocational nursing program
Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
Must possess and maintain valid CPR certification while employed in a clinical role
Must be a licensed driver who can travel to all business locations

Enters patient data per established procedures in the electronic medical record; maintains accurate and continued nursing documentation. Observes general and specific physical and behavioral signs and symptoms; assesses for, and effectively communicates, deviations from normal conditions; recognizes urgent/emergent situations and refers them to a RN or other provider as appropriate. Provides direct patient care, evaluates outcomes, and consults RN, MD, and FNP as needed; notifies appropriate staff of changes in the patient’s condition. Monitors patient’s vital signs; demonstrates knowledge of cardiac monitoring and identifies arrhythmias.
