Position Overview

We are seeking a skilled LPN to join our nursing team. In this role, you will provide high-quality patient care, perform treatments, administer medications, and support residents’ overall health and comfort. You will also collaborate with CNAs to ensure timely, accurate care and maintain thorough documentation.

Key Responsibilities

Patient Care & Support

  • Promote and restore residents’ health through the nursing process

  • Perform treatments and procedures as prescribed

  • Provide physical, educational, and emotional support to residents and families

  • Supervise assigned CNA team members to ensure quality care

Medication Administration & Reporting

  • Safely administer medications according to physician orders

  • Give and receive relevant reports, both written and verbal, from appropriate staff

Documentation & Compliance

  • Maintain accurate, complete resident care documentation using electronic medical records

  • Ensure CNA documentation is accurate and timely

Qualifications

  • Current, valid NY LPN license

  • CPR Certification

  • Experience in a nursing home or hospital setting preferred

  • Strong teamwork, communication, and organizational skills
