Looking for a passionate and compassionate RN to assumes responsibility for individual medical case management of agency clients, on a case-by-case basis. Responsible for ensuring the quality of medical care provided meets DDD and Agency standards while working cooperatively with the CBS agency administrative staff

 

Responsibilities;

  • Monitors the health of the individuals through review of Professional Medical Reports.
  • Assesses the educational needs of the staff and conducts or arranges for appropriate in-service training programs.
  • Advocates in the best interest of the clients at all times, acting as a medical liaison between staff and hospital/medical professionals when necessary.
  • Maintains the confidentiality, security and integrity for the management of client information according to agency and DDD policies
  • Communicates and collaborates with other team members and community agencies in planning, implementing and coordination and evaluating all services necessary to insure comprehensive care.
  • Performs other duties as may be assigned by Director of Residential Services and/or the Executive Director.

Candidate must be able to work on-call schedule (one week on, one week off) Current NJ RN License in good standing Hospice Experience Preferred Willing to travel to the assigned areas Must have reliable transportation with liability insurance Strong communication and interpersonal skills.
