About the Role
We are seeking an experienced Wellness Director (LVN) to serve as the lead community nurse for a senior living community in Chico, CA. This role is responsible for directing all aspects of resident care, ensuring clinical compliance with RCFE regulations, and leading a large caregiving team while partnering closely with residents, families, and leadership.
Key Responsibilities
Direct and oversee all resident care and assisted living services
Evaluate resident care needs and determine appropriate levels of care
Manage medication administration programs, audits, and pharmacy coordination
Maintain resident charts, assessments, and support plans
Oversee clinical compliance with RCFE, Title 22, and state regulations
Track and manage immunization and health records for residents and staff
Review incidents and implement interventions to reduce risk
Order and maintain medical supplies and equipment
Ensure safe operation of all health-related equipment
Communicate changes in resident condition to physicians, families, and leadership
Implement and oversee staff orientation, in-services, and required annual training
Supervise, evaluate, and support a team of 30–45 caregivers
Interview and evaluate job applicants as needed
Participate in community operations and assist in care roles when necessary
Operate the department within budget
Communication & Leadership
Maintain strong, professional relationships with residents, families, staff, physicians, and vendors
Handle difficult conversations with families regarding care needs and rate increases
Promote teamwork, accountability, and a positive work environment
Uphold resident rights, confidentiality, and HIPAA compliance
Report suspected abuse, neglect, or misappropriation per state requirements
Requirements (Must-Have)
Active, unencumbered California LVN license
Minimum 1 year experience in assisted living or skilled nursing
Clean driving record
Ability to pass DOJ/FBI background check
Reliable transportation and valid vehicle insurance
Valid CPR and First Aid certification
Strong leadership, organization, and communication skills
Preferred Experience
Elder care experience required
Assisted Living and/or Memory Care experience strongly preferred
Experience managing large caregiving teams
Familiarity with RCFE and Title 22 regulations
Working Conditions
Ability to work required schedules including evenings, weekends, holidays, and overtime as needed
Must meet physical demands including standing, lifting, bending, and responding to emergencies
