Job Description

  • Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
  • May be responsible for the admissions of new residents to include all admission assessments and documentation.
  • Responsible for completing acute and long-term care plans or service plans.
  • May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
  • Provide general consultation regarding health-related concerns within the community.
  • Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
  • Respond quickly to all emergencies.
  • Maintain close communication with all departments and staff members.
  • Provide educational meetings for residents and staff.
  • Understand roll in the Safety & Disaster Plan.
  • Maintain confidentiality of residents’ and prospective residents’ information.    
  • May be responsible for discharges, assessments, and clinical documentation as needed. 

Qualifications

Level of Formal Education: Training from an accredited licenses or vocational nursing program 
Area of Study: General nursing 
Years of Experience: 1+ years
Type of Experience: Senior care or related field
Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing.  Also, needs to be CPR Certified
Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations.
Technical Competencies: Basic computer skills 
Skills and Ability: 

  • Ability to work as part of a team and handle multiple tasks safely and effectively.

Personal Attributes: high tolerance for working under pressure and handling critical situations in a calm and professional manner

Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Verifies electronic submissions of MDS, performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. Assists disciplines in formulating and revising care plans. Ensures that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. Evaluates resident care plans for comprehensiveness and individuality. Assesses the achievement or lack of achievement of desired outcomes. Ensures that resident’s care plan is reassessed and revised appropriately. Responsible for all level of care changes within the facility. Notifies all departments when a level of care change has been made. Generates appropriate forms to complete level of acuity and changes. Transmits forms to the appropriate agency for processing as required by state law. Other duties as assigned. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
