Acts is currently seeking qualified candidates for our Wellness Home Services Licensed Practical Nurse (LPN) position.

In this role, you will be responsible for delivering individualized, one-on-one care tailored to each resident’s unique needs, under the direction of a Registered Nurse (RN), where required by state guidelines. Specializes in managing chronic conditions, supporting post-operative recovery, and addressing complex medical needs with a steadfast commitment to safety, dignity, and comfort. Performs medication management, vital sign monitoring, and collaborates with multidisciplinary care teams, while providing clear education and guidance to residents and families. Committed to delivering exceptional, high-quality care that improves health outcomes, promotes independence, and enhances overall quality of life.

Requirements

The ideal candidate will meet the following requirements:

Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing
Minimum of 1-2 years of clinical nursing experience, preferably in acute care or home health setting
Current CPR certification (or obtain within three months of employment)
If responsible for transporting a resident, must maintain a valid state driver’s license and provide proof of current automobile liability insurance

Pay Range

$23.69 - $28.75 / hour. Starting rate will vary based on skills and experience.

Responsibilities Verify plan of care with the RN prior to each visit. Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes. Teach patients and caregivers self-care techniques. Check vital signs. Review all medications and perform a medication reconciliation at each visit. Perform specialized procedures ordered by physician (i.e. foley catheter change). Prepare equipment and materials for treatments using the aseptic technique. Complete documentation timely and accurately. Qualifications One year of clinical LPN experience.
