The Wound Care RN provides expert nursing assessment, planning, implementation, and evaluation of wound care for residents in a skilled nursing facility. This role ensures evidence-based wound management, promotes skin integrity, and collaborates closely with interdisciplinary team members, physicians, and support staff to achieve optimal healing outcomes. The RN also serves as a clinical resource and educator on wound care protocols across the facility.
Key Responsibilities
Clinical Assessment & Care

Conduct comprehensive wound and skin assessments for residents, including pressure injuries, diabetic ulcers, surgical wounds, and chronic non-healing wounds.

Develop, implement, and evaluate individualized wound care plans based on current evidence-based protocols.

Perform and oversee wound treatments including wound cleansing, debridement, dressing selection/changes, negative pressure therapy (if applicable), compression therapy, and other advanced modalities as ordered.
Documentation & Quality Assurance

Maintain accurate, timely, and compliant documentation in the medical record of assessments, interventions, wound measurements, and resident responses.

Monitor trends in wound healing, report changes in condition, and adjust care plans in collaboration with the interdisciplinary team.

Participate in facility quality improvement activities related to skin and wound care outcomes.
Interdisciplinary Collaboration

Collaborate with attending physicians, nurse practitioners, dietitians, therapy services, and other clinical staff to coordinate wound management strategies

Lead or participate in weekly wound rounds and high-risk skin reviews.
Education & Training

Educate facility nursing staff, CNAs, residents, and families on wound care techniques, prevention strategies, and appropriate product use.

Serve as a clinical resource for staff to enhance overall wound care competency and compliance with regulatory standards.
Regulatory Compliance

Ensure wound care practices adhere to facility policies, state regulations, and national standards

Participate in wound care-related audits, infection control practices, and survey readiness activities.
Qualifications
Required
-
Current Registered Nurse (RN) license in the State of Wisconsin.

Basic Life Support (BLS) certification.
-
Demonstrated clinical nursing experience, preferably including wound care or long-term care.

Strong assessment, critical thinking, and communication skills.
Preferred

Certified Wound Care Nurse (CWCN) or similar specialty certification.

Experience in wound care settings, geriatric care, or post-acute/long-term care.

Knowledge of current wound care products, pressure injury prevention, and evidence-based practices.
