PRN RN Case Manager/Inpatient Services
About the Role:
The PRN - RN Case Manager plays a critical role in coordinating and managing patient care through the continuum, on an as-needed basis, within the acute medical units at Wooster Community Hospital. This position focuses on assessing patient needs, developing individualized discharge plans, and facilitating communication among healthcare providers, patients, and families to ensure optimal health outcomes. The RN Case Manager will monitor patient progress, advocate for appropriate services, and assist in navigating complex healthcare systems to reduce hospital readmissions and improve quality of care.
Working in a flexible, PRN capacity, the nurse will respond to fluctuating patient volumes and provide expert clinical judgment to support continuity of care. Ultimately, this role ensures that patients receive timely, efficient, and comprehensive care tailored to their unique needs.
Minimum Qualifications:
- Current and valid Registered Nurse (RN) license in the United States.
- Bachelor's Degree
- 5-years of experience in an acute care setting.
- No written disciplinary action with the last 12 months.
- Strong knowledge of healthcare systems, insurance processes, and community resources.
- Excellent communication and interpersonal skills to effectively collaborate with diverse patient populations and healthcare teams.
- Proficiency in electronic health record (EHR) systems and documentation standards.
Preferred Qualifications:
- Prior case management experience in a hospital or related healthcare setting.
- Certification in Case Management (CCM) or equivalent credential.
- Experience working in a PRN or flexible scheduling environment.
- Knowledge of discharge processes and resource utilization.
- Familiarity with chronic disease management.
- Education in Motivational Interviewing techniques

- Employee in this position may be exposed to blood, body fluids and/or airborne pathogens and is required to wear barriers. .
Responsibilities:
- Conduct comprehensive patient assessments to identify medical, psychosocial, and environmental needs.
- Develop, implement, and update individualized care plans in collaboration with multidisciplinary teams.
- Coordinate communication between patients, families, healthcare providers, and community resources to facilitate seamless care transitions.
- Monitor patient progress and outcomes, adjusting care plans as necessary to meet evolving needs.
- Advocate for patient rights and access to appropriate healthcare services while ensuring compliance with regulatory standards.
- Document all case management activities accurately and timely in the electronic health record system.
- Provide education and support to patients and families regarding plans of care, medications, and available resources.
- Assist in discharge planning to reduce hospital readmissions and promote patient safety.
- Respond flexibly to staffing needs by providing case management services on an as-needed basis.
Skills:
Strong communication skills are essential for coordinating multidisciplinary teams and ensuring clear, compassionate interactions with patients and families. Proficiency with electronic health records supports accurate documentation and efficient information sharing across care settings. Preferred skills such as case management certification and experience with chronic disease management enhance the nurse's ability to navigate insurance and community resources effectively. Together, these skills ensure the RN Case Manager can deliver high-quality, patient-centered care while adapting to the dynamic demands of a PRN role.
8 hours shifts as needed Monday - Saturday; On-call rotation for Sundays and Holidays, rotating basis
As needed - 8 hour shifts, 7:45 AM - 4:15 PM
Hours may require flexibility based on departmental needs.
