About the Role

The MDS Coordinator oversees the RAI/MDS process, ensures accurate and timely assessments, and collaborates closely with all clinical departments. You will play a key role in maintaining compliance with CMS and LTCI guidelines and supporting resident-centered care planning.

Key Responsibilities

Complete and process all MDS assessments accurately and on time
Collaborate with nursing staff and interdisciplinary teams to develop person-centered care plans
Communicate effectively with all departments to ensure timely RAI/MDS transmission
Review and prepare reports for Quality Measures and Quality Indicators
Maintain compliance with federal and state regulations
