Hospice RN Inpatient Admission Coordinator

Position Summary
The Hospice RN Inpatient Admission Coordinator serves as the primary connection between Affinity Care of Ohio, referral sources, patients, and families. This role is responsible for building strong community relationships, identifying hospice-appropriate patients, coordinating admissions, and ensuring a compassionate, seamless transition into hospice services. The ideal candidate is a skilled communicator with a deep understanding of hospice philosophy and a commitment to patient-centered care.
Key Responsibilities
Referral & Relationship Management

Build and maintain strong relationships with hospitals, skilled nursing facilities, physicians, social workers, and other referral partners.

Educate healthcare professionals about hospice philosophy, services, eligibility criteria, and Affinity Care of Ohio’s programs.

Respond promptly to new referrals and inquiries, ensuring timely follow-up and exceptional customer service.
Admissions Coordination

Assess hospice eligibility by reviewing clinical information, communicating with referral sources, and consulting with the hospice medical director.

Conduct in-person or virtual patient and family meetings to explain hospice services, benefits, and expectations.

Coordinate and complete all admission paperwork, consents, and required documentation in compliance with Medicare, Medicaid, and state regulations.

Ensure smooth, timely transitions by collaborating with the clinical team, scheduling nurses, and communicating key patient needs.
Clinical & Operational Support

Communicate effectively with interdisciplinary team members to ensure continuity of care.

Provide education to families regarding hospice philosophy, symptom management, and available support services.

Maintain accurate, timely documentation in the EMR system.

Participate in team meetings, case discussions, and performance improvement initiatives.
Qualifications

Required:
-
Experience in hospice, palliative care, home health, or related healthcare field.

Strong understanding of Medicare hospice regulations and eligibility criteria.

Excellent communication, interpersonal, and presentation skills.

Ability to manage time effectively and respond to urgent situations.

Valid driver’s license and reliable transportation.

Sales or community outreach experience in healthcare settings.
