Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs
Implement the individualized plan of care and revisions as needed
Compliance with AdvisaCare Policies and Procedures, Medicare compliance
Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
Supervision of paraprofessionals providing services to patient
Preparing clinical and progress notes, participating in IDG conferences
Participate in Home Health and Hospice performance improvement program
Maintain high level of customer satisfaction with our clients, families and caregivers
Communication of information using current process and technology within organization

Requirements:

Current Michigan License in good standing
Minimum 1 year home health and hospice experience
Empathetic approach to patients and caregivers
Participate in field visits daily, weekly office meetings as indicated
Ability to adapt to patient’s status and needs

Medical Office Nurse RN - OB/GYN (West Branch)

Summary Under direct supervision of the specialty provider, will be responsible for patient assessment and care, and maintaining appropriate documentation. This position must have knowledge of specialty care and able to assess, plan, implement and evaluate care for patients. This position is responsible for coordinating care to assure high quality, compassionate patient care is provided. They will maintain and oversee patients at the office locations and will act as the liaison with other health care providers, organizations and departments. This position may be required to order all medical, pharmacy supplies, and office supplies to manage patient care. They will assure compliance with all the policies/procedures of MyMichigan Health and follow Occupational Safety and Health Administration (OSHA) guidelines, Clinical Laboratory Improvement (CLIA) regulations, and all appropriate regulatory agencies. They will coordinate with and train any new employees, students and float staff as required. The clinical person will provide patient education, assist with specialty care modalities, symptom management, be available for emergencies, monitor for compliance and the effects of care management with high risk patients, educate on prescriptions and teach the patient and family the reasons for the medication and side effects, coordinate and ensure continuity of care with other providers.
