Job Description:

JOB SUMMARY

The Licensed Practical Nurse (LPN) is a vital member of the patient care team.  In collaboration with the RN, the LPN provides direct patient care and education consistent with the patient’s plan of care and desired outcomes.  The LPN collects and communicates pertinent information to the interdisciplinary team and completes routine and delegated activities.

EDUCATION

For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

Minimum Required: Knowledge of patient care standards and practices acquired through graduation from an accredited LPN program or documentation of nursing education and training through an accredited Registered Nurse program that meets state requirements to take LPN boards.

Preferred/Optional: Licensed practical nurse (LPN) diploma.  Associate degree in nursing.

EXPERIENCE 

Minimum Required: Experience working with patient care.  Demonstrated proficiency with verbal, non-verbal and written communication skills.

Preferred/Optional: Experience as a licensed practical nurse within an area relative to the hiring department.

CERTIFICATIONS/LICENSES
