Staffing Response Team (SRT) RN – Level IV (Day Shift) - Intermediate Care

Ideal Experience:
This position is ideal for nurses with Intermediate Care experience. Initially, teammates will work primarily in the areas listed below. After six months, and based on departmental needs, nurses may be trained to work in additional areas to further expand their skills and versatility.

Primary Clinical Areas:

  • Cardiovascular Surgical Stepdown

  • Cardiac Medicine Unit

  • Cardiac Procedural Medicine Unit

  • Medical Stepdown

  • Intermediate Care

Work Locations:
The ideal teammate must be willing to float between UNC Rex Raleigh and UNC Rex Holly Springs campuses. Daily assignments are based on the operational needs of both facilities.

Role Requirements:
Teammates in this role are required to work a minimum of two clinical groupings, which include:

  • Medical and Surgical (MedSurg) – counts as two clinical groupings

  • Intermediate Care

  • Critical Care

  • Emergency Department – counts as two clinical groupings

If your experience is in one clinical grouping but not another, we offer training and orientation opportunities to help you meet the requirements of this position.

Shift Requirements:

  • Day shift: 7:00 AM – 7:00 PM

  • Level IV commitment: Minimum of 24 hours per week (two 12-hour shifts) during each six-week scheduling period

  • Minimum of two 12-hour weekend shifts per six-week scheduling period

  • Required to work two official holidays and one unofficial holiday annually

Summary:

Performs assessments, documents patient status, develops individualized plan of care, implements plan of care, evaluates status, and initiates patient education plans according to individualized needs of patient.


Responsibilities:
1. Maintains an environment of safety for patients, self and others and supports patient safety initiatives. Utilizes PPID correctly. Responds promptly to all alarms. Follows fall prevention and skin care protocols. Utilizes the Safe Patient Handling equipment appropriately.

4. Documents patient's assessment, care provided and response to care according to procedure. Assures that all tasks delegated to others are documented correctly. Provides reports in a thorough and concise manner and utilizes SBAR format. Assesses patient's condition, including: biophysical, psychosocial and age specific needs.
5. Develops an individualized plan of care utilizing the nursing and medical diagnosis. Collaborates with other health care team members in formulating plan of care. Initiates the plan of care upon admission. Orders appropriate tasks in RCARE. Initiates consults and serves as an advocate to meet the patient's needs.
6. Implements the nursing plan of care on assigned patients. Reviews all diagnostic data and responds appropriately. Maintains a calm manner and works effectively at all times. Implements nursing interventions to prevent complications.
7. Evaluates patient's status, nursing instructions, appropriateness of the nursing plan of care and outcomes of the care provided. Updates and resolves the plan of care as appropriate.
8. Clarifies medication orders, as needed, prior to the end of shift. Medications are administered using two identifiers: patient name and medical record number. High alert medications are verified by a second nurse prior to administration. Medications are explained to the patient (name, action, side effects) prior to administration.
9. Initiates a patient's education plan according to the individualized needs of the patient, as prescribed by the physician and/or hospital procedure; includes patient and family instruction, as well as, discharge planning. Documents instruction and education to patient and family.
10. Recognizes, supports and utilizes evidence based nursing practice. Is aware of and follows Rex policies, procedures and protocols. Attends and completes all mandatory education.
11. Adheres to the American Nurses Association Code of Ethics, as demonstrated by professionalism, commitment to the patient, and accountability and responsibility for individual nursing practice.

Clinical Nurse II OR - (Per Diem) Endoscopy

UNC Johnston Endoscopy is a fast-paced procedural unit located in Smithfield. It is a hospital-based endoscopy department. There is an enormous number of opportunities for medical professionals to grow, learn and develop into GI experts. Currently we operate up to 2 procedural rooms daily: with an average case load of 12 cases per day. We have cultivated an environment where team camaraderie and peer support translate to a healthy and strong work culture. Our hours of operation are Monday- Friday from 7:30 – 4pm with some rotating on call time Monday – Friday after hours of operation. We also have a phenomenal team of doctors. You will have the opportunity to learn from some of the best and become part of the team that delivers exceptional services for patients undergoing endoscopic procedures such as, colonoscopies, upper endoscopy, esophageal and colon stents, manometries, the capsule, ERCPs and bronchoscopies. Our nurses are trained to work in each of the patient care areas (pre, intra and post procedure). Rest assured, you will receive excellent training and will be well equipped to perform these various roles. We are a passionate group who takes great pride in delivering quality services. The team is dedicated and committed, but most of all, patients are our top priority. Reporting and accountable to the Nurse Manager/Director, the Clinical Nurse is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting. Responsibilities: 1. Education - Participates in identifying and meeting learning needs of self. Attends education programs based on identified learning needs. With assistance, uses patient education materials relevant to patient population. Assesses readiness to learn of the patient/ family/ caregivers. Assesses developmental level of patient and factors affecting ability to learn. 2. Evaluation of Care - Identifies expected patient outcomes. Seeks guidance as needed in revision of plan of care. Participates in discussion with members of the interdisciplinary team in evaluation of patient care. Communicates relevant information to promote continuity of care 3. Implementation - Demonstrates competence in care of patients with complex problems, including population-appropriate physical, psychosocial, educational and safety aspects of care. Performs bedside point of care testing as required for patient care plan. Utilizes healthcare organization and nursing standards, policies and procedures in delivery of care. Organizes and prioritizes care according to patient/family needs. Consults with appropriate resources in a timely fashion regarding patients with complex care issues, unusual teaching needs and/or those at high risk for discharge planning. 4. Leadership - With assistance, develops goals to promote professional growth or minimize limitations. Achieves goals and objectives within identified time frame or renegotiates with supervisor. Is knowledgeable about activities which facilitate intra/interdepartmental collaboration. Participates in development and achievement of unit goals and performance improvement activities. Effectively uses communication systems. Participates in promoting cost-effective care. Gives feedback to co-workers. Is aware of public policy and regulatory guidelines affecting the health care environment. Promotes a safe, clean and secure hospital environment for all 5. Patient Assessment - Recognizes data from complex situations to determine priorities for care. Includes appropriate physical, psychosocial, education and safety needs. Synthesizes assessment data into meaningful whole prior to communication to others. Assesses and anticipates discharge needs of individual patients and families. Develops relationships with families that promote their ability to advocate for the patient and their own needs. 6. Planning - Collaborates with patient/family to prepare or update the plan of care. Makes use of available multidisciplinary resources in planning care. Begins to use full range of communication as a means to convey planning. Identifies and addresses cultural and ethnic issues in planning patient care. Demonstrates ability to prioritize tasks for patients with complex problems. 7. Research - Reads journals that contain studies or articles that may be applicable to practice. Brings ideas and questions to the staff at large for assessment of applicability. Licensure/Certification Requirements: ● Licensed to practice as a Registered Nurse in the state of North Carolina. ● Basic Life Support (BLS) for Healthcare Provider certification Professional Experience Requirements: ● Two (2) years of nursing experience.
