Position Overview 

As a Home Care Registered Nurse (RN) at Trinity Health at Home and Mercy Home Health, you’ll deliver one-on-one, high-quality care to patients in the comfort of their homes. Using advanced technology and your clinical expertise, you’ll assess, plan and manage individualized care that promotes healing and independence. 

This position is based out of our office on South Broad Street and will cover NORTH PHILADELPHIA.

Minimum Qualifications 

Graduate of an accredited nursing program

Active RN license in the Commonwealth of Pennsylvania

Minimum of one (1) year of professional nursing experience

Employment Type: Full time Shift: Night Shift Description: Trinity Health Grand Rapids' Hauenstein 3 (H3) is an innovative 34-bed mixed acuity Neuroscience Medical Surgical Unit able to care for low acuity up to non-intubated critical care patients. On H3, we pride ourselves on the neuroscience nursing specialty, focusing on the unique expertise needed to care for this specialized patient population. The nurses on this unit care for a wide range of patients who have undergone complex neurological surgeries and procedures as well as trauma. We are also a designated Comprehensive Stroke Center and therefore care for newly diagnosed stroke patients in need of emergent thrombolytic and neuro interventional treatment. Additionally, H3 contains an Epilepsy Monitoring Unit which provides these patients all types of medical and surgical care options. On H3, specialized training and education is provided during a comprehensive orientation and available thereafter continually for neuro nursing expertise. Position Summary: Registered Nurse, RN works in a collaborative environment to deliver excellent patient care as part of an interdisciplinary team providing evidenced based medicine and individualized patient care.
